We have this Ford in town - the only one of its kind I have seen. Don't know the model year.

It's a mystery. Sits up high but has an E badge instead of an F.

Either way - it seems to spend most of its time at one repair shop or the other.

That is definitely an extended E350 Ford Van with a custom rear cap from the cab to the rear.  The front isnt the same as a HD Ford F350.  Extended E350's were common for shuttles or cargo vans.  The van in the photo still has the twin opening side doors just like a stock E350, they just extended them to the new roofline.
